What Is BHRT — and Is It Right for You?

By Stephanie Zwonitzer, DNP | Revive Institute of Sexual Health

Let's be honest for a second. You've probably Googled "hormone replacement therapy" at some point — maybe at 2am when you couldn't sleep (again), or after your third hot flash of the day left you questioning everything. And what came up was probably a confusing mix of outdated studies, horror stories from the early 2000s, and a lot of conflicting opinions.

So let's clear the air.

Bioidentical Hormone Replacement Therapy — BHRT — is one of the most misunderstood, and honestly, most life-changing tools in modern medicine. And at Revive Institute of Sexual Health, it's the foundation of almost everything we do. So whether you're a woman navigating perimenopause, a man quietly wondering why you feel like a shadow of yourself, or someone who just knows something is off — this one's for you.

First Things First: What Even Is BHRT?

Bioidentical hormones are hormones that are chemically identical to the ones your body naturally produces. We're talking estrogen, progesterone, testosterone, DHEA — the whole cast of characters that run the show behind the scenes of your energy, mood, metabolism, libido, and more.

The "bioidentical" part matters because these hormones are derived from natural plant sources (typically yams or soy) and are structurally identical to human hormones at a molecular level. This is different from synthetic hormones used in traditional HRT, which are similar but not identical — and that difference matters to how your body recognizes and uses them.

Think of it this way: bioidentical hormones are like a key that was made specifically for your lock. Synthetic hormones? They're close — but not quite the right fit.

Why Do Hormones Get Out of Balance in the First Place?

Here's the thing nobody warned you about: hormonal decline is completely normal. It starts earlier than most people realize and affects everyone — not just women going through menopause.

For women, estrogen and progesterone begin shifting in your 30s and early 40s during perimenopause — the years leading up to menopause. Once you hit menopause (defined as 12 consecutive months without a period), those levels drop significantly and stay there, making you wonder where the woman you’ve always been has gone.

For men, testosterone peaks in your 20s and then quietly declines about 1–2% per year after age 30. By the time most men notice something is off, their levels may have dropped significantly from their peak.

Add in the effects of chronic stress (which tanks your cortisol and throws everything else off), poor sleep, processed diets, and environmental factors — and you've got a recipe for hormonal chaos at almost any age.

What Does Hormone Imbalance Actually Feel Like?

This is where it gets real. Because hormonal imbalance rarely shows up as one obvious symptom. It shows up as a collection of things that make you feel like you're slowly becoming a lesser version of yourself.

For women, this might look like:

  • Hot flashes and night sweats that are ruining your sleep (and making more laundry!)

  • Mood swings, anxiety, or a low-grade depression that just won't lift

  • Brain fog — forgetting words, losing your train of thought mid-sentence

  • Weight gain, especially around your midsection, despite doing "everything right"

  • Low libido, vaginal dryness, or pain during sex

  • Fatigue that no amount of coffee fixes

For men, it often shows up as:

  • Persistent fatigue and low energy

  • Decreased motivation or drive — that spark just feels… gone

  • Difficulty building or maintaining muscle

  • Increased body fat, especially around the belly

  • Low libido or erectile dysfunction

  • Mood changes, irritability, or depression

Sound familiar? You're not imagining it. And no, it's not just "getting older." It's hormones — and hormones can be addressed.

How Does BHRT Actually Work?

At Revive, we don't guess. Every patient starts with a comprehensive consultation and lab work to get a clear picture of exactly where your hormone levels are. From there, we create a personalized protocol — because your hormones are as unique as you are, and your treatment should be too.

BHRT can be delivered in several ways depending on what works best for your body and lifestyle: creams, gels, patches, or injections. We'll talk through the options and find the right fit for you.

Once we start, we monitor closely. Hormones aren't a "set it and forget it" situation — they require regular check-ins and adjustments, especially in the early months. That's exactly why our programs at Revive are structured the way they are: to give you real support throughout the process, not just a prescription and a "good luck."

Is BHRT Safe?

This is the question everyone asks — and rightfully so, given the confusion that's been circulating for decades.

The short answer: yes, BHRT is considered safe for most people when properly prescribed, monitored, and individualized. The bad reputation around hormone therapy largely stems from a 2002 study (the Women's Health Initiative) that used synthetic, non-bioidentical hormones at doses and delivery methods that don't reflect current practice. The medical community has evolved significantly since then.

That said, BHRT isn't for everyone. Certain health histories — like some types of hormone-sensitive cancers — may change the conversation. That's why a thorough intake, lab work, and an honest discussion about your personal health history are non-negotiable before we ever write a prescription. Your safety is the whole point.

Who Is a Good Candidate for BHRT?

Great question. You might be a good candidate if you:

  • Are a woman in perimenopause or menopause experiencing symptoms that affect your quality of life

  • Are a man experiencing symptoms of low testosterone (also referred to as andropause or "low T")

  • Have had lab work showing hormone levels that are low or out of optimal range (did someone look at them and say “you’re normal?”)

  • Feel like your energy, mood, libido, or mental clarity have significantly declined

  • Want a personalized, root-cause approach to how you're feeling — not just a band-aid

Age isn't the only factor here. We see patients as young as their mid-30s who are already experiencing hormonal shifts. The right time to explore BHRT is when your quality of life is being affected — full stop.

What BHRT Is NOT

Let's also be clear about what we're not doing here. BHRT is not a magic pill. It's not a substitute for sleep, nutrition, movement, and stress management — all of which we talk about as part of your whole-person care at Revive. Hormones work best when the foundation underneath them is solid.

And BHRT is not one-size-fits-all. Cookie-cutter dosing based on age alone is not how we operate. Your protocol is built around you — your labs, your symptoms, your goals, your lifestyle.

So... Is BHRT Right for You?

That's honestly what we're here to find out — together. At Revive Institute of Sexual Health, every new patient starts with a $250 consultation where we go deep: your history, your symptoms, your past labs, your goals. From there, we'll know whether BHRT may be the right path.  Then we’ll update your labs and determine exactly how to approach it for your body and your goals.

You don't have to keep white-knuckling through fatigue, mood swings, brain fog, or a sex drive that's gone MIA. There is another option — and it's personalized, science-backed, and delivered by someone who genuinely gives a damn about how you feel.
