Waking at 2 a.m. soaked, losing words mid-sentence, missing closeness. These are hormonal signals, and they can be measured.

Estrogen and progesterone can start shifting in your late 30s or 40s, years before your last period. You do not have to wait it out.

What Is BHRT for Women?

BHRT (bioidentical hormone replacement therapy) uses hormones that are molecularly identical to the estrogen, progesterone and testosterone your body makes. For women, it is used to ease symptoms of perimenopause and menopause, such as hot flashes, poor sleep, mood changes, low desire and vaginal dryness, when your own levels decline.

The word “bioidentical” describes the molecule, not a brand or a single product. Many FDA-approved prescriptions are bioidentical. We start there, and we tell you plainly which option you are getting and why.

FDA-approved bioidentical

Standardized doses with established safety data. Usually our first choice. Available as patches, gels, creams, capsules and vaginal preparations.

Compounded, when needed

Custom-mixed by a pharmacy when an approved product does not fit, such as an allergy to an ingredient. Compounded hormones are not FDA-approved and are not proven to be safer.

Care for Every Stage

Often mid 30s to early 50s

Periods still come, but you feel different

Progesterone often falls first while estrogen swings. That can mean heavier or unpredictable cycles, broken sleep, anxiety and night sweats. Standard labs can look “normal” here, so we read your symptoms and your numbers together.

Twelve months after your last period

Hot flashes, sleep and focus take the hit

Estrogen and progesterone settle at lower levels. Hot flashes, night sweats, brain fog, joint aches and vaginal dryness are common. Starting hormone therapy before 60 or within ten years of menopause tends to offer the most favorable balance of benefit and risk.

The years that follow

Comfort, intimacy and long-term health

Some symptoms fade while others, like vaginal dryness and bladder changes, can grow. Local vaginal hormone therapy, bone and heart markers, and a plan that changes with you all stay in view.

Is BHRT Safe?

For many healthy women under 60 or within ten years of menopause, the benefits of hormone therapy for symptom relief outweigh the risks. The right answer for you depends on your age, timing, personal and family history, and the type of hormone used.

In November 2025 the FDA announced it would remove the broad boxed warnings from menopausal hormone therapy labels, reflecting newer research on timing and risk. A warning remains for estrogen used without progesterone in women who have a uterus, which is why we pair them when you need both.

Hormone therapy is not right for everyone. A history of certain cancers, blood clots, stroke or liver disease may change your options.

How we keep it careful

Care That Listens First

Led by Dr. Stephanie Anderson, DNP

Revive Institute of Sexual Health was built to bridge the gap between rushed traditional visits and what women in midlife actually need: time, real data and a provider who knows your case. Here your concerns are heard, your labs are analyzed and your treatment is individualized.

Hormones and intimacy, together

Hormone changes reach your relationships too. When desire, comfort or connection shift, we treat that as part of your health, with options for Couples Care when you want your partner involved.

What is BHRT for women?

BHRT, or bioidentical hormone replacement therapy, uses hormones that are molecularly identical to the estrogen, progesterone and testosterone your body makes. For women it is used to ease perimenopause and menopause symptoms such as hot flashes, night sweats, poor sleep, mood changes, low desire and vaginal dryness. At Revive, treatment starts only after a full evaluation of your symptoms, history and labs.

Common signs include hot flashes or night sweats, waking in the night, brain fog, new anxiety or irritability, low libido, vaginal dryness or painful intimacy, irregular or heavier periods, joint aches and fatigue that sleep does not fix. These can start in your late 30s or 40s. A Foundation consult helps sort out which symptoms are hormonal and which have another cause.

For many healthy women under 60 or within ten years of menopause, the benefits of hormone therapy for symptom relief outweigh the risks. In November 2025 the FDA announced it would remove the broad boxed warnings from menopausal hormone therapy labels, while keeping a warning for estrogen used without progesterone in women with a uterus. Your own risk depends on your age, timing, history and the type of hormone, so we review all of it first and monitor you over time.

Bioidentical describes the molecule, not the quality of a product. Several FDA-approved prescriptions are bioidentical and come with standardized doses and safety data, so they are usually our first choice. Custom compounded hormones are not FDA-approved and are not proven to be safer or more effective. We use them only when an approved option does not fit, and we always tell you which one you are getting.

Sometimes. Testosterone can help some postmenopausal women with persistently low sexual desire. There is no FDA-approved testosterone product for women in the United States, so its use is off-label. We consider it only after a full evaluation, at doses meant for women, with follow-up labs.

Revive is a membership practice and does not bill insurance. Care begins with the Revive Foundation, two comprehensive appointments, and continues with the Vitality Membership for ongoing monitoring and adjustments. Our team walks you through pricing before you commit, and we provide documentation for HSA or FSA reimbursement where it applies. Lab and pharmacy costs depend on where you have them done.

Many women notice changes in hot flashes and sleep within a few weeks of starting treatment. Mood, energy and vaginal comfort can take two to three months, and doses are often fine-tuned during that time. Individual results vary.

Our providers are licensed in Maryland, so care is available to women who live in or are physically located in Maryland at the time of their visits. Visits happen over secure video, labs are drawn at a site near you, and prescriptions go to a Maryland pharmacy or are delivered.

No referral is needed. You can book your Foundation consult directly. If you have recent labs, bring them. If you need new ones, we coordinate testing at a draw site convenient to you anywhere in Maryland.
